1. Piece and purpose

Private piece ID; short description; report recipient; insurance, sale, estate, or other purpose.

2. Assignment requirements

Requested effective date; recipient requirements; value basis to confirm with the appraiser.

3. Evidence supplied

Photograph filenames; receipts; laboratory report numbers; provenance documents; previous appraisal date.

4. Known changes and uncertainties

Repairs, replacement stones, resizing, condition issues; distinguish family recollection from documentation.

5. Appraiser and scope

Name/contact; relevant credentials; inspection method; laboratory work; fee and completion date.

6. Custody and report questions

Intake receipt; transport/storage arrangements; assumptions or estimates that need explanation.

7. Completion

Report received/date; description checked; recipient acceptance; next review trigger.
