# Health evidence checklist

Use: read one research or regulatory claim and prepare questions for a qualified clinician. This worksheet does not diagnose, recommend treatment or establish eligibility. Do not include identifiable health records in a publicly shared copy.

## Claim and source
Claim in my own words:
Original study or regulatory document URL:
Publication date and date checked:
Evidence type (laboratory, model, clinical study, authorization, implementation):

## What was studied?
Population, exclusions and setting:
Intervention or test:
Comparison group or method:
Sample size and follow-up:
Outcome actually measured:
Absolute results in each group:
Uncertainty and missing information:
Harms, withdrawals and practical burdens:
Funding and conflicts reported:
Independent confirmation:

Fictional arithmetic: 20 events per 100 versus 15 per 100 is a 5-percentage-point absolute difference and a 25% relative reduction. These invented numbers describe no real treatment.

## What would it mean in care?
Country and exact authorized use, if applicable:
Who the evidence applies to:
What a result would change:
Follow-up required:
Availability, cost and coverage questions:

## Questions for a clinician
Is this relevant to the person's circumstances?
What alternatives and uncertainties should we consider?
What would be the next step after each possible result?
What benefit, harm and practical burden should be discussed?

Next evidence milestone or review date:
